Redesigning Care for Populations with Complex Needs and High Costs: Operationalizing the Institute of Healthcare Improvement’s Triple Aim

The Institute for Healthcare Improvement (IHI) works with over 140 healthcare organizations worldwide to facilitate improvement in care, cost, and health with a particular focus on populations with complex health and social needs. Organizations that thoroughly assess patient needs are able to co-design care approaches with patients to improve health while driving down total cost. IHI’s work demonstrates that, in many cases, better care designs and greater patient and community engagement can result in financially sustainable programs that improve health outcomes at lower cost.

Over the past two years, the Hospital Home Teams (HHTs) of the WRHA have participated in IHI’s Better Health and Lower Costs (BHLC) Triple Aim Collaborative. They, alongside a host of other teams situated across Canada, the United States, and Europe have actively applied IHI’s methodology and adopted IHI’s “all teach, all learn” philosophy to redesign care for patients with complex health and social needs.

Learning Objectives:

  • Gain understanding of IHI Methodology and the “all teach, all learn” philosophy
  • Learn about the team’s experience of redesigning care for patients with complex health and social needs using IHI methodology and why they were named “one of the top teams in the BHLC Collaborative”
